This preregistration form is to be filled out and submitted before first class, by those who wish to participate in our programs (introductory trial lessons or otherwise). This is preliminary paperwork to get you started, and separate student member paperwork will need to filled out at the studio to be an official member of the school.

We will never disclose your email address. Your address will be used for communication purposes only, between you and staff of Seattle Wushu Center.

On your first lesson, please arrive 10 to 15 minutes before your class time, to fill out a participation form.
